Southgate Health Care Center's specialized long-term care facility has become the area leader for inpatient physical and nursing rehabilitation. Our 152 bed skilled nursing facility offers a refreshing alternative to the institutional hospital setting in our modern 84 bed subacute and transitional care unit.
The Transitional Living Center at Southgate offers comprehensive sub-acute rehabilitation services for those who suffer from a variety of medically complex conditions.
We strive for quality healthcare with a focus on improving the body, mind and spirit. Our emphasis is on quality care with a personal touch. For the convenience of our patients and families, we arrange medical diagnostic tests and specialty services (ie. laboratory, x-ray, pharmacy, podiatry, mental, optometry, ophthalmology) to be provided and/or performed on-site whenever possible.
When our patients are not participating in therapy, they are able to recuperate in a home-away-from-home environment. We offer lovely, comfortable accommodations that are spacious enough to enjoy visits from loved ones. Other amenities, such as cable, telephone service, and a full service beauty salon are available.
A wide range of activities, pastoral care, and spiritual services are available to meet each individual patient's needs.
Skilled nursing and rehabilitation services are provided by a dedicated interdisciplinary team with experience in the subacute setting.
Nursing rehabilitation, including I.V. therapy, pain management, would care, and respiratory services, is provided by qualified nursing personnel. Our physical, occupational, and speech therapy services are available on a full time, on-site basis.
The Transitional Living Center's innovative physical and nursing rehabilitation programs have been successful in returning patients to their home environment.
Our staff, along with the attending physician, will assess each patient to establish their current functional status and identify their needs for nursing and physical rehabilitation.
Physiatrists (medical doctors specializing in physical medicine and rehabilitation) also assess patients, coordinate therapy services, and participate in rehab team conferences to assure complete coordination of care.
Our Interdisciplinary team, which includes nurses, social workers, dieticians, therapists, and activities professionals work collaboratively to develop personalized treatment programs that include medical care, nutrition management, as well as a broad range of therapeutic activities.
We recognize that family members and caregivers are an integral part of the patient's recuperation. Our interdisciplinary team encourages participation and feedback from family and caregivers in this interdisciplinary process.
Therapists will conduct an initial evaluation to determine each patient's specific impairments; this helps us develop goal-oriented outcomes and discharge planning expectations for the patient and/or family. Depending on each individual's needs, therapy services are available in our 2,500 square foot, state-of-the-art rehabilitation center.
This unique training area, including a fully functional kitchen, bedroom, and bathroom simulates the home environment.
